Most people are primarily concerned with finding caregivers to care for their parents in their homes with a seamless system.  Our licensed professionals can help you navigate the complex medical system.

      • Care Transition Coordination A transition of care is when a patient moves between care settings, such as when a patient leaves the hospital and returns home or goes to a rehabilitation facility. Care transitions also occur when a patient’s condition or situation changes. We provide the coordination during care transitions that helps prevent problems such as medication errors or failure to transfer important information.
      • Coordination & Supervision Of In-Home Care & Companion Services When home care is required for elderly or disabled persons, we are happy to refer families to trusted providers. Regardless of the referral source, however, home caregivers are entrusted to work otherwise unsupervised in patients’ homes. To ensure peace of mind, our team can conduct regular, unannounced visits to accurately review the quality of care being provided.

        Where a loved one will reside is one of the most common dilemmas caregivers face.  Dr. Kasmir will help you strategically plan for all facets of aging to maximize asset preservation, legal protection, and overall health (all issues of particular concern when seniors require assisted living facilities). In the event assisted care is needed, either for rehabilitation or long- term care, we can avoid the common missteps often made unintentionally by loving family members and improve the chances of admission to a preferred facility.

        One of our primary objectives is to ensure the best possible care for every patient while involving community resources to alleviate caregivers’ stress and financial burdens. These professional resources may include government, non-profit, and private programs. We often find that our fees are offset by the community resources and entitlement benefits we discover for our patients
      • End-of-Life Counseling & Hospice Planning
        Many seniors want to discuss end-of-life issues with a trusted companion outside of the family circle.  Dr. Kasmir is uniquely positioned to have frank conversations with dying patients and are able to facilitate the difficult decisions that patients and families must jointly make.
